1.     Topic re: Accept donations totaling $8,296.78 to the Community Services Agency Fund. – Community Services

2.     Topic re: Approve Berlin Midget Football to sell food and beverages at their home football games at Scalise Football Field, Sage Park for the 2012 fall season. – Parks and Recreation

3.     Topic re: Approve Berlin Dolphin Swim Team Booster Club to sell food and beverages at Berlin Dolphin Swim Team home meets at Percival Pool this summer. – Parks and Recreation

4.     Topic re: Approve the transfer of $6,250 from the Child and Youth Services revenue account to the Youth Grant expenditures account. – Youth Services

5.     Topic re: Approve the Nursing Department application to the Marjorie Moore Fund for a grant to provide Homemaking and Bath Services for a total of $15,000. - VNA

6.     Topic re: Accept a donation in the amount of $50.00 by Sam’s Club for use by the Hatchery Brook Community Garden. – Development Services

NEW BUSINESS:

1.     Topic re: Approve the appropriation of an additional $274,586 to Excess Special Education Grant Revenue and Excess Special Education Grant Expenditure. – Board of Education

2.     Topic re: Approve waiving the bid process and award the contract to Qualidigm for an amount not to exceed $25,000, since this the best interest of the town. - VNA

3.     Topic re: Send awarding a lease agreement with VNA Healthcare to lease five telemonitors with daily monitoring by a cardiac nurse for amount not to exceed $12,700 to a Public Hearing on June 19, 2012 at 6:45 p.m. - VNA

4.     Topic re: Approve Metro Swim Shop to sell swimsuits, etc. on Tuesday, June 26, 2012, 6:00 p.m. - 7:30 p.m. at Percival Pool to participants on the Berlin Swim Team and to waive the vendor fee of $200.00. – Parks and Recreation

5.     Topic re: Approve waiver of light fee of around $222.00 for Berlin Women’s Softball League for usage of Hubbard Field during the month of August 2012. – Parks and Recreation

6.     Topic re: Accept the bid and award the radio system maintenance contract to Utility Communications and authorize the Town Manager to execute this contract with the single payment option of $7,102.20. – Police Department

7.     Topic re: Approve the transfer of $2,500.00 from the Physicals Account and $2,000.00 from the Traffic Account for a total transfer of $4,500.00 to the Misc. Equipment Account. – Police Department

8.     Topic re: Approve the transfer of $12,000.00 from the Computer Support Account and $3,000.00 from the Radio Maintenance Account for a total transfer of $15,000.00 to the Rifle Account. – Police Department

9.     Topic re: Approve a transfer for $13,445 from Golf Cart Lease into the following accounts: $2,650 into Part time & Summer Help, $1,690 into Starters, Rangers and Golf Cart Hauling, $6,300 into Electric,  $2000 into Maintenance and Repairs and $805 into water and sewer.  The total transfer will not to exceed $13,445. - Golf Course

10.  Topic re: Approve the donation of four eighteen hole rounds of golf, with carts, at Timberlin Golf Course to be used as raffle prizes for the Hospital of Central Connecticut Auxiliary fund raising golf tournament being held at Tunxis Plantation on June 5, 2012. The value of the donation is $210. – Golf Course

11.  Topic re: Approve utilizing CRCOG Bid #568 and authorize award for the procurement of line painting to the lowest qualified contractor in accordance with the provisions of the CRCOG Contract. – Public Works

12.  Topic re: Refer the matter of roadway widening improvements on Orchard Road to the Planning and Zoning Commission for an 8-24 Review (as per the CT General Statutes). – Public Works

13.  Topic re: Approve utilizing the State D.A.S. Contract #11PSX0332 for the installation of guide rails and enter into a contract with the lowest qualified Contractor. – Public Works

14.  Topic re: Report out to the Town Council regarding the Berlin High School renovation project. – Public Works

15.  Topic re: Approve the request by Carleen Quinn to waive the site plan application fee of $195.00 for a proposed Farmer’s Market. – Development Services

16.  Topic re: Approve the new Hatchery Brook Community Garden Fee Structure and to amend the Town’s Fee Schedule for 2012 and 2013 as recommended by the Conservation Commission to be as follows:  5’ x 10’ plots will be $25.00; 8’ x 20’ plots will be $30.00; 10’ x 25’ plots will be $35.00; and, 20’ x 20’ plots will be $40.00.  There will be an additional $10 fee charged for non residents. – Development Services

17.  Topic re: Approve the request of the Economic Development Commission to waive the greens fees and carts fees in the amount of $840.00 for the Society of Industrial and Office Realtors and golf event scheduled for June 14, 2012. – Economic Development

18.  Topic re: Appropriate the $500,000 State of Connecticut Regional Brownfield Grant to the State and Federal Grants revenue account and to the Environmental Remediation expenditure account. – Economic Development

19.  Topic re: Transfer funds from the Wage Negotiation and Townwide fringe benefit accounts into various departmental Longevity and fringe benefit accounts.  Total amount of transfer will be $19,392. – Town Manager
