CONSENT AGENDA:

1.     Topic re: Accept donations totaling $150.00 for Berlin-Peck Memorial Library and deposit to the Agency Fund from which related purchases will be made. – Berlin-Peck Memorial Library

2.     Topic re: Accept a donation of $100.00 from the Whole Donut of Berlin and deposit the funds into the Special Grants and Donations Fund and appropriate the funds to the Donations Revenue and Miscellaneous Equipment Expenditure Account to help pay for miscellaneous equipment needs not budgeted for in the general fund budget. – Police Department

3.     Topic re: Authorize the Town Manager to enter into a farm use License Agreement (in the form per the attached sample) with Ryan Matson for the portion of the property across from Berlin High School on Patterson Way that is appropriate for farm use. – Town Manager

NEW BUSINESS:

1.     Topic re: Presentation by Paul Hutcheon, Director of Health, with the Central Connecticut Health District on the Community Health Survey. – Health District

2.     Topic re: Approve the Timberlin Golf Course rates for the 2012 golf season. – Golf Course

3.     Topic re: Appropriate $41,500 from the Capital and Nonrecurring Fund Revenue Account, Sale of Labor, Land, and Materials and transfer such funds to the Vehicle Reserve Account for the future purchase of vehicles, equipment and related items. – Municipal Garage

APPOINTMENTS:

  1. Board of Ethics Vacancy - Term would be until January 31, 2014. Can be filled with a D, R or U.
  1. Board of Ethics – William Watson III (D) resigned on January 4, 2012. Replacement term would be until January 31, 2014. Can only be filled with an R or U if above appointment is filled with a D.
  1. Board of Ethics – Alternate – Vacancy – Term would be until January 31, 2014. Can be filled with an R or U.
  1. Conservation Commission – James Matulis’s (D) term expires on January 31, 2012. Reappointment or replacement term would be until January 31, 2017. Can be filled with a D, R or U.
  1. Environmental Protection Advisory Commission (New Commission adopted by Town Council 7/5/2011) – Shall consist of seven (7) members, three of which have already been appointed at the 9/20/11 council meeting (Carolyn Wysocki (D) term ending 1/31/2012, Barbara Coughlin (D) term ending 1/31/2013, and Neil McCarthy (D) term ending 1/31/2013). Four members still need to be appointed - one member shall be appointed to serve until January 31, 2013; and three (3) members shall be appointed to serve until January 31, 2014. Thereafter, all members shall be appointed to serve a term of three (3) years.
  1. Historic District Commission – Susan Skene’s (D) term expires on January 31, 2012. Reappointment or replacement term would be until January 31, 2017. Can be filled with a D, R or U.
  1. Inland Wetlands & Water Courses Commission – Edward Hrubiec III’s (U) term expires on January 31, 2012. Reappointment or replacement term would be until January 31, 2015. Can be filled with a D, R or U.
  1.  Inland Wetlands & Water Courses Commission – Fred Pindar’s (R) term expires on January 31, 2012. Reappointment or replacement term would be until January 31, 2015. Can only be filled with a D, R or U.
  1. Inland Wetlands & Water Courses Commission Alternate – Vacancy – Term would be until January 31, 2014. Can be filled with an R or U.
  1. Mattabassett District – John Luddy's (D) term expired August 31, 2011. Replacement term would be until August 31, 2014. Can be filled with a D, R or U.
  1. Plainville Area Cable Television Advisory Council (PACTAC) – Vacancy – Term expired on June 30, 2011. New Term will expire June 30, 2013. Can be filled with a D, R or U.
  1. Plainville Area Cable Television Advisory Council (PACTAC) – Vacancy – Term expired on June 30, 2011. New Term will expire June 30, 2013 Can be filled with a D, R or U depending on the appointment above.
  1. Planning & Zoning Commission – Frederick J. Sorbo IV’s (U) term expires on January 31, 2012. Reappointment or replacement term would be until January 31, 2017. Can be filled with a D, R or U.
  1. Planning & Zoning Commission Alternate – Vacancy - Term would expire on January 31, 2014. Can be filled with a D, R or U.
  1. Planning & Zoning Commission Alternate – Vacancy – Term would expire on January 31, 2016. Can only be filled with a D or U if above appointment is filled with an R.
  1. Public Building Commission – John Dunham’s (R) term expires on January 31, 2012. Reappointment or replacement term would be until January 31, 2017. Can be filled with a D, R or U.
  1. Berlin VNA – Nancy Baccaro (D) resigned January 10, 2012. Replacement term would until January 31, 2014. Can be filled with a D, R or U.
  1. Water Control Commission Alternate – Vacancy Term would expire on January 31, 2014. Can be filled with a D, R or U.
  1. Water Control Commission Alternate – Lenny C. Michaud’s (U) term expires on January 31, 2012. Reappointment or replacement term would be until January 31, 2015. Can be filled with a D, R or U.
  1. Youth Services Advisory Board Service Consumer – Vacancy – Term would expire on January 31, 2014.
